Sick (fit) notes

If you are off work for up to seven days in a row (including weekends and bank holidays), you do not need to give your employer a fit note from a medical professional.

You can self certify. During this time you should complete a SC2 self-certification form which is available from your employer or you can fill it in online and download it from the HMRC website by clicking the button below:

Only after being off work with sickness for seven days in a row will you need to request a fit note from a medical professional. We will only produce a fit note after you have self certified for the first seven days.  To request a consultation for a fit note from a clinician, click the button below and follow the steps:

When Your Fit Note Runs Out

If your certificate runs out, but you are still sick, you will need to consult the doctor again before you can get a further certificate.

Fit certificates can be back-dated so it is not necessary to make an emergency appointment to renew your certifiate. If you attend an emergency appointment for the purpose of getting a medical certificate you will be asked to rebook.
